I authorize 'The Shoreline' Church and/or it's agents to make an independent investigation of my background, references, character, past employment, education, criminal, or police records, including those maintained by both public and private organizations and all public records for the purpose of confirming the information contained on my application and/or obtaining other information, which may be material to my qualifications for employment/ministry now, and if applicable, during the tenure of my employment/ministry with 'the shoreline' Church. I release 'the shoreline' Church and/or it's agents and any person or entity, which provides information pursuant to this authorization, from any and all liabilities, claims, or lawsuits in regards to the information obtained from any and all of the above referenced sources used. The following is my true and complete legal name and all information is true and correct to the best of my knowledge.
I recognize that as a minister, my life and actions will be examples of Christ-likeness. I am living my life, with God's help and to the best of my abilities, in accordance with God's word and His Grace. *
More specifically, I am living my life within the boundaries that God has set: I am not living with anyone of the opposite sex with whom I am not married (Eph. 5:3);
I am not involved in a sexual relationship outside of marriage (Acts 15:20; 1 Cor. 10:8);
I am not involved in an adulterous relationship (Ex. 20:14; Matt. 5:27);
I am not living a homosexual lifestyle (Lev. 18:22; 1 Cor. 6-9);
I am not involved in the misuse or abuse of alcohol (Eph. 5:18);
I am not involved in addictive behavior, including, but not limited to: illegal drugs, alcohol, or pornography (1 Cor. 6:12; Eph. 5:18).

The information contained in this application is correct to the best of my knowledge. I authorize any references or churches listed in this application or verbally throughout the remainder of this process to give you any information (including opinions) that they may have regarding my character and fitness serving as a minister. In consideration of the receipt and evaluation of this application by 'the shoreline' Church of San Clemente, I hereby release any individual, church, youth organization, charity, employer, reference, or any other person or organization, including record custodians, both collectively, and individually, from any an all liability for damages of whatever kind of nature which may at any time result to me, my heirs, or my family, on account of compliance or any attempts to comply, with this authorization. I waive any rights that I may have to inspect any information provided about me by any person or organization identifiied by me in this application. Should my application be accepted, I agree to be bound by the policies and conduct covenants of 'the shoreline' Church, and to refrain from unscriptual conduct in the performances of my services on behalf of the church. I further state that I HAVE CAREFULLY READ THE FOREGOING RELEASE AND KNOW THE CONTENTS THEREOF AND I SIGN THIS RELEASE AS MY OWN FREE ACT. I acknowldege that this is a legally binding agreement and I have read and understaood it in it's entirety.
