Tides: All About My Child

Thank you for entrusting your child to us. We strive to provide for every child spiritually and physically while they are at The Shoreline Church. This questionnaire will give our volunteers more information when working with your child so that we can provide a safe environment and a good experience for your child. The Shoreline Church respects your family’s right to privacy, any information shared from this form is communicated directly with those caring for your child and only on a “need-to-know” basis.
